If it can’t be fixed, a “Reinitialize Base Stations” button in PiTools would be a useful workaround.
I don’t seem to have an issue with my single base station.
The only way to “reinitialize” basestations is to power them off and on again.
Anyway, it looks like a problem with the particular install/configuration and not a general problem, as I do not need to delete or reinitialize anything.
As it turns out, neither did I, even though that was the prevailing suggestion. In my case, just moving the headset very close to the base station and then slowly moving it away was the solution. I have to note that the problem seems worse with the latest “beta” Pimax drivers. I have to repeat the procedure numerous times, but it does eventually work.

As @Heliosurge suggests You should add an option to restart the service from PiTool. Maybe a right-click option on the tray icon?
That seems like a reasonable suggestion. They already added a reboot button for the HMD button in PiTool, so I would expect they will get around to adding this.

Did you try to just stop-wait-start the pilauncher service ?
